Dr. Miller: When do you treat a fissure or hemorrhoids with a procedure?
Dr. Pickron: Ill talk about the fissures first and probably maybe 10 or 20% of those are actually treated surgically. We have creams that we can use that generally take care of things. If that doesnt work, Botox injection is an option. If those dont work, then the last resort is certainly surgery.
Dr. Miller: So thats treatment for fissures, how do you treat hemorrhoids?
Dr. Pickron: Again, most of these are treated non-operatively. There are procedures in the office we do. The most common thing we do is a procedure called rubber band ligation, where we put rubber bands around the hemorrhoids on the inside. That sounds pretty painful, but truthfully the internal hemorrhoids really have no sensation, so its a very easy procedure to do and very well tolerated. Its usually just some mild pressure for about 24 hours and thats it. The ones that do need to be treated surgically, like I said, are typically the external ones and those are, fortunately for patients, fairly rare.
Dr. Miller: It would seem to me to be rare, but are there times that the pain could be induced by something like a cancer?
Dr. Pickron: It is. Thats certainly one reason that these symptoms need to be evaluated very thoroughly.
Dr. Miller: So if theyre going on for weeks, thats something that probably ought to be looked at?

The solution often surprises many people, which is splash some water on it.
Rather than cleaning yourself with flushable wipes, use a bidet a water spray which can you install on your toilet, or even a portable, spray bottle version or a plastic sitz bath which is available at any drug store. Pat dry after that and do it gently.
Whatever method you choose, rinse the area with water to clean it, then pat dry, Dr. Zutshi says. Dont use soap and water, unless its very mild like Ivory® or Dove® the chemicals in soap dry the area too. Just plain water works fine.
My patients tell me that after they started doing this, their itching and discomfort went away, she says.
You can also use a barrier cream, like Desitin® or Calmoseptine® , when experiencing discomfort and then later, as needed, to calm the area.

If your diagnosis includes hemorrhoids, treatment may start immediately. In the event of multiple hemorrhoids, often the largest, most symptomatic hemorrhoid is banded first. Additional appointments are then scheduled to treat the remaining hemorrhoids at two week intervals with a final check-up two or more weeks after the last remaining hemorrhoid is banded.
