Hemorrhoids are swollen veins. Each of us has veins around the anus that tend to stretch under pressure. One set of veins is inside the rectum and another is under the skin around the anus . Hemorrhoidal tissue is thought to be helpful in holding back stool at rest and in cushioning the sphincter muscles when we empty our bowels.When these veins swell and bulge, they are called hemorrhoids or piles. Swelling can be caused by straining to move your bowels, from sitting too long on the toilet, or from other factors such as pregnancy, obesity or liver disease.

Symptoms

The most common symptom of internal hemorrhoids is bright red blood covering the stool, on toilet paper or in the toilet bowl. Bleeding starts when the swollen veins are scratched or broken by straining or rubbing. This can be aggravated by medicines that thin the blood. An internal hemorrhoid may protrude through the anus outside the body, becoming irritated and painful. This is known as a protruding or prolapsed hemorrhoid.

Symptoms of external hemorrhoids may include painful swelling or a hard lump around the anus that results when a blood clot forms. This condition is known as a thrombosed external hemorrhoid.

How Serious Are My Hemorrhoids

Treatment will depend on your symptoms, but typically,doctors will recommend conservativeoptions first.

Mild or occasional rectal bleeding may respond to changes in dietary habits, like upping your dietary fiber and drinking more water, Dr. Zutshi says. For periodic anal irritation caused by inflamed hemorrhoids, over-the-counter skin protective creams and warm sitz baths may help.

There are also some simple, in-office treatments for mild, on-and-off bleeding from internal hemorrhoids. Rubber band ligation is a commonly used procedure that involves placing little rubber bands above a hemorrhoid to cut off the blood supply. The hemorrhoid then shrinks and falls off.

This procedure is usually only used on smaller hemorrhoids,Dr. Zutshi explains. But larger internal hemorrhoids can still be easilytreated with minimally invasive surgery.

If the issue is large internal and external hemorrhoids, a more invasive surgery might be necessary. However, if its only skin tags, which are residual hemorrhoid tags, these are usually not treated, Dr. Zutshi says, as trimming them off is very painful, and residual bulges may still persist.

Occasionally, external hemorrhoids become very painful whenthey develop a clot in them, she explains. This is called a thrombosed hemorrhoid.If you see your doctor within a few days of onset, the clots can be removed,Dr. Zutshi says. Otherwise it takes a few weeks for the swelling and pain tosettle down.

Stapled hemorrhoidectomy is the newest addition to the armamentarium of surgical internal hemorrhoid procedures. It has several aliases, including Longo’s procedure, the procedure for prolapse and hemorrhoids , stapled circumferential mucosectomy, and circular stapler hemorrhoidopexy.

Stapled hemorrhoidectomy is mostly used in patients with grade III and IV hemorrhoids and those who fail prior minimally invasive treatments. During stapled hemorrhoidectomy, a circular stapling device is used to excise a circumferential ring of excess hemorrhoid tissue, thereby lifting hemorrhoids back to their normal position within the anal canal.

Stapling also disrupts hemorrhoid blood supply. Studies have suggested that stapled hemorrhoidectomy results in less postoperative pain and shorter recovery compared with conventional surgery, but a higher rate of recurrence. Frequency of complications is similar to that following standard hemorrhoidectomy.
