In Grade II, hemorrhoids protrude past the anal verge as you strain during the bowel movement but spontaneously return to its original internal position once the straining has subsided. They are larger than Grade 1 hemorrhoids.

Hemorrhoids that are hidden inside the rectum are called internal hemorrhoids. Their presence is unseen, but they can cause symptoms. Evidently, hemorrhoids that you can see or feel are external hemorrhoids.

What Causes A Hemorrhoid To Become Prolapsed
A hemorrhoid can become prolapsed when the tissue that holds it in place weakens. There are several possible causes and risk factors for this weakening of the connective tissue.
Straining during bowel movements is one possible cause, as the straining can put extra pressure on the hemorrhoid. You may be more likely to strain if youre experiencing constipation or diarrhea.
Pregnancy can also increase your risk. Hemorrhoids occur in up to 40 percent of pregnant women, and left untreated, they can become prolapsed.
Obesity is another possible risk factor. Excess weight can put a strain on the rectal veins, causing the formation of hemorrhoids and the prolapse of internal and external hemorrhoids.
Cigarette smoking can also harm any and all of your blood vessels, including the veins in your rectum and anus. That can increase your risk for hemorrhoids and prolapsed hemorrhoids.
Differences From Other Grades
Hemorrhoids are classified based on their degree of prolapse.
- Grade 1: The least severe internal hemorrhoids, grade 1 hemorrhoids are not prolapsed. Rather, they are entirely contained inside the rectum.
- Grade 2: These hemorrhoids do extend out of the anus during a bowel movement, but they return to their normal position on their own.
- Grade 3: These hemorrhoids can be manually shifted back into place.

How To Diagnose Grade 2 Hemorrhoids
The diagnosis of grade 2 piles or internal hemorrhoids is difficult for most patients. Your doctor can verify if you have hemorrhoids by asking about your symptoms or with a physical examination.
Your doctor may do a digital rectal examination with a gloved finger inside your rectum. This is called an anoscopy, where your doctor inserts a short, lighted scope into your anus and test the inside of your anal canal.
The doctor may recommend a colonoscopy if the person with piles presents signs and symptoms that suggest underlying digestive diseases, or they are demonstrating any risk factors for colorectal cancer.
Diagnosing And Classifying Hemorrhoids
Hemorrhoids are the distal prolapse of the arteriovenous bundle, muscle fibers and surrounding connective tissue as an envelope below the dentate line in the anal canal. They usually present with painless rectal bleeding.2

Prolapsed hemorrhoids are not generally accompanied by pain unless they become thrombosed or strangled. If you are experiencing pain from prolapsed hemorrhoids, there are many pain relief options available. These include:

If the patient does not get enough fiber, it is important to consider dietary changes and add a supplement. Not all fiber supplements work the same, so it is important try different brands or types. For example, Metamucil gets mixed with a full glass of water and requires eight additional glasses of water a day. If the patient is not a regular water drinker, fiber gummies are a good alternative. The goal is for a patient to have a soft, easy-to-pass stool . Patients can start with one fiber gummy a day and work up to five safely to achieve the desired outcome. Benefiber, another viable option, can be mixed with other foods such as mashed potatoes or even soup.
As mentioned before, fiber without enough water may cause constipation, so consider the patients medical history and fluid volume. Fiber supplements can also cause a patient to bloat and have increased gas, but these symptoms tend to subside after a week of therapy while the body adjusts to it.
Observation Within One Week Postoperatively
All patients had postoperative VAS scores ranging from 1-6, and the two groups of data did not conform to a normal distribution. The control group had a VAS score of 3 and the observation VAS of 4 . Mann-Whitney U test showed no significant difference in VAS scores between the two groups of patients .
As shown in Tables 2, 3 and 4, 55 patients in the control group did not experience urinary retention, 46 no postoperative bleeding, and 58 didn´t have anal marginal edema 58 patients in the observation group did not experience urinary retention, 55 had no postoperative bleeding, and 60 cases had no anal marginal edema. According to the evaluation criteria, patients urinary retention, postoperative hemorrhage, and wound marginal edema were classified according to grades I-IV. Mann-Whitney U test showed that there was no significant difference in the occurrence of urinary retention, bleeding, and wound edge edema in two groups of patients .

A conservative measure is the mainstay for the treatment of hemorrhoids in this group of patients. If required, injection sclerotherapy appeared to be a better and safer alternative to banding and hemorrhoidectomy for treating bleeding hemorrhoids. Antibiotic prophylaxis is always given before performing any intervention, even a minor office-based procedure, due to the possibility of bacteremia.
Differences Between Other Grades
Internal hemorrhoids are divided into four classifications, and grade 4 hemorrhoids are the most severe. They are rated based on their level of prolapse.
While grade 1 hemorrhoids do not protrude from the anus at all, grade 2 poke out during a bowel movement and then return to their normal location.
As hemorrhoids worsen, their prolapse is harder to resolve. Grade 3 hemorrhoids require manual manipulation to return them to their spot. Grade 4, the most severe, do not reduce at all. They are permanently prolapsed, and even pushing them back in place will not keep them inside the rectum.

Can Hemorrhoids Be Treated Without Surgery
A very common question that I am asked is whether everyone with hemorrhoids needs surgery. And the truth of the matter is that only a minority of patients need surgery for hemorrhoids. Many times, I am actually able to treat hemorrhoids more effectively in the office than in the operating room. So, who can be treated in the office and who needs surgery for hemorrhoids?
First, we should talk about the difference between external and internal hemorrhoids. Ive talked about this difference in another post as well. External hemorrhoids have pain nerves and cant be treated with hemorrhoid banding. Imagine if you put a rubber band on your finger it would hurt thats why you cant rubber band external hemorrhoids. But, internal hemorrhoids dont have pain nerves. They have nerves known as autonomic nerves, which supply sensations similar to the intestinal tract . Because internal hemorrhoids dont have pain nerves, they can be treated with procedures in the office.
Theres a grading scale for hemorrhoids. Its useful to understand this so we can talk about different types of hemorrhoids:
* First Degree Bleed only, but do not prolapse
* Second Degree Bleed, prolapse, but spontaneously reduce
* Third Degree Bleed, prolapse, and require manual reduction
* Fourth Degree Bleed, prolapse, and are irreducible

A number of OTC creams, ointments, and pads that contain hydrocortisone or witch hazel are available to help relieve pain, swelling, and itching caused by hemorrhoids.
