Hemorrhoids can happen inside or outside the rectum. The type depends on where the swollen vein develops. Types include:

  • External: Swollen veins form underneath the skin around the anus. Your anus is the canal where poop comes out. External hemorrhoids can be itchy and painful. Occasionally, they bleed. Sometimes they fill with blood that can clot. This is not dangerous, but can result in pain and swelling.
  • Internal: Swollen veins form inside the rectum. Your rectum is the part of the digestive system that connects the colon to the anus. Internal hemorrhoids may bleed, but they usually arent painful.
  • Prolapsed: Both internal and external hemorrhoids can prolapse, meaning they stretch and bulge outside of the anus. These hemorrhoids may bleed or cause pain.

Is It A Hemorrhoid Or Something Else

Dr. Pickron: Most of the patients we see for anorectal pain don’t have hemorrhoids, but everybody has hemorrhoids to some degree.

Dr. Miller: Most don’t.

Dr. Pickron: Most don’t, but the real causes of pain are more things like fissures, an abscess, and other things that aren’t hemorrhoids.

Dr. Miller: Fissures can be incredibly painful.

Dr. Pickron: They’re horrible.

Dr. Miller: What causes a fissure?

Dr. Pickron: Usually it’s a tear and it happens directly in the front or directly in the back. Most of the time these things heal up just fine, but if they don’t and they get deep enough to where the anal sphincter gets irritated . . .

Dr. Miller: Ouch.

Dr. Pickron: . . . then you get a spasm of that muscle and that prevents the tear from healing and then you just enter this vicious cycle that just won’t quit.

Dr. Miller: So you use different therapies to try to repair that.

Dr. Pickron: Yeah, generally we focus not so much on the tear but on the muscle itself because if we could make the muscle relax and stop the spasm then the tear generally heals just fine on its own.

  • Sigmoidoscopy: Your provider uses a sigmoidoscope to view inside the lower part of the colon and rectum. Procedure types include flexible sigmoidoscopy and rigid sigmoidoscopy .

These tests may be uncomfortable but arent painful. They typically take place in a doctors office or outpatient center without anesthesia. You go home the same day.

Your provider may perform a colonoscopy to confirm findings from other tests or check for signs of colon cancer. This outpatient procedure requires anesthesia.

How To Ease Hemorrhoid Pain While Pooping

Constipation is one of the leading causes of hemorrhoid pain. For those with external hemorrhoids, bowel movements can be particularly painful.

To ease the pain associated with hemorrhoids, include more fiber in your diet to ensure your stool is soft. Drink plenty of fluids and exercise regularly. Avoid laxatives, as they can make your hemorrhoids worse.

Dont wait too long to use the bathroom. If you need to have a bowel movement, waiting to go will cause your stool to become drier and harder.

Practice proper hygiene, especially after your bowel movement. Rinsing in the shower can be helpful to both hygiene practices and easing the pain of wiping.

Avoid straining during your bowel movements. Elevating your feet can also be very helpful.

Place a step stool at the base of your toilet to change the position of the rectum and allow easier stool passage.

What Should I Do If A Hemorrhoid Bursts

A burst hemorrhoid usually doesnt require any treatment. But you may want to take a sitz bath to soothe the area and keep it clean while it heals. A sitz bath can also help to increase blood flow to the area, which aids the healing process.

To take a sitz, bath, follow these steps:

  • Fill a clean bathtub with 3 to 4 inches of warm water make sure its not too hot.
  • Soak the area for 15 to 20 minutes.

How To Cure Hemorrhoids

Dr. Miller: When do you treat a fissure or hemorrhoids with a procedure?

Dr. Pickron: I’ll talk about the fissures first and probably maybe 10 or 20% of those are actually treated surgically. We have creams that we can use that generally take care of things. If that doesn’t work, Botox injection is an option. If those don’t work, then the last resort is certainly surgery.

Dr. Miller: So that’s treatment for fissures, how do you treat hemorrhoids?

Dr. Pickron: Again, most of these are treated non-operatively. There are procedures in the office we do. The most common thing we do is a procedure called rubber band ligation, where we put rubber bands around the hemorrhoids on the inside. That sounds pretty painful, but truthfully the internal hemorrhoids really have no sensation, so it’s a very easy procedure to do and very well tolerated. It’s usually just some mild pressure for about 24 hours and that’s it. The ones that do need to be treated surgically, like I said, are typically the external ones and those are, fortunately for patients, fairly rare.

Dr. Miller: It would seem to me to be rare, but are there times that the pain could be induced by something like a cancer?

Dr. Pickron: It is. That’s certainly one reason that these symptoms need to be evaluated very thoroughly.

Dr. Miller: So if they’re going on for weeks, that’s something that probably ought to be looked at?

What Causes Hemorrhoids

Traditionally, hemorrhoids are associated with chronic constipation, straining during bowel movements, and prolonged sitting on the toilet all of which interfere with blood flow to and from the area, causing it to pool and enlarge the vessels. This also explains why hemorrhoids are common during pregnancy, when the enlarging uterus presses on the veins.

More recent studies show that patients with hemorrhoids tend to have a higher resting anal canal tone that is, the smooth muscle of the anal canal tends to be tighter than average . Constipation adds to these troubles, because straining during a bowel movement increases pressure in the anal canal and pushes the hemorrhoids against the sphincter muscle. Finally, the connective tissues that support and hold hemorrhoids in place can weaken with age, causing hemorrhoids to bulge and prolapse.

For most people, the signs and symptoms of hemorrhoidsitchiness, swelling, discomfortwill be mild and respond to home remedies. In a minority of cases, however, hemorrhoid signs and symptoms will be severe enough that they require treatment from a physician. Rarely, hemorrhoids that cause significant blood loss or a blood clot can be life-threatening. It’s important to note that one of the chief calling cards of hemorrhoids, blood in or on the stool or on toilet paper, should never be considered normal. While the cause could indeed be hemorrhoids, a more serious condition could be afoot.

Hemorrhoids can come with external and/or internal symptoms, depending on how they form.

External Hemorrhoids

Hemorrhoids on the outside of the anus are known as external hemorrhoids. A hard lump might be felt in that area and can often be uncomfortable because it is irritated by bowel movements and by wiping with toilet paper. External hemorrhoids are painful because of the many nerve endings located on the skin around the anus.

If the hemorrhoid becomes inflamed and engorged with blood it may look bluish-purple and be painful, not only during a bowel movement but even when walking or sitting.

Different Kinds Of Hemorrhoids Can Cause Similar Symptoms

When internal hemorrhoids do progress, they will often protrude outside the anus, becoming prolapsed hemorrhoids. An internal hemorrhoid that has reached this stage can cause some external hemorrhoid-like symptoms, such as itchiness and swelling. Since prolapsed hemorrhoids and external hemorrhoids can cause similar symptoms, its sometimes difficult to know which is which without a doctors professional diagnosis. It is also not uncommon for both to be present at once.

Different Types of Hemorrhoids

There are two types of hemorrhoids: external and internal. External hemorrhoids arise from the anal canal, while internal hemorrhoids arise from the rectum, immediately above the anal canal. According to Cuckoo Choudhary, MD, associate professor of gastroenterology at Thomas Jefferson University in Philadelphia, one type is not worse than the other, but both can cause problems. “It all depends on the size and degree of the hemorrhoid,” Dr. Choudhary says. “Untreated internal hemorrhoids can cause bleeding. External hemorrhoids can cause thrombosis , which gives way to severe pain from hemorrhoidal strangulation.”

If you know you have hemorrhoids and you have acute and severe anal pain, it could be a sign of thrombosed hemorrhoids.

Know When to See Your Doctor

Though the annoyance of hemorrhoids can be reason enough to call your doctor, use this checklist to know when a visit is a must:

  • If you’re experiencing any type of rectal bleeding
  • If the hemorrhoids are causing you pain or discomfort
  • If the problems persist despite trying over-the-counter hemorrhoid creams or other remedies
  • If you’re passing stools that look maroon in color or tarry in color, a sign of bleeding

If you experience a large amount of rectal bleeding that is accompanied by dizziness or faintness, be sure to seek emergency medical care immediately.
