There are two types of hemorrhoids: external and internal. External hemorrhoids arise from the anal canal, while internal hemorrhoids arise from the rectum, immediately above the anal canal. According to Cuckoo Choudhary, MD, associate professor of gastroenterology at Thomas Jefferson University in Philadelphia, one type is not worse than the other, but both can cause problems. “It all depends on the size and degree of the hemorrhoid,” Dr. Choudhary says. “Untreated internal hemorrhoids can cause bleeding. External hemorrhoids can cause thrombosis , which gives way to severe pain from hemorrhoidal strangulation.”
If you know you have hemorrhoids and you have acute and severe anal pain, it could be a sign of thrombosed hemorrhoids.
Know When to See Your Doctor
Though the annoyance of hemorrhoids can be reason enough to call your doctor, use this checklist to know when a visit is a must:

How Do You Treat A Bleeding Hemorrhoid
Home treatment
Acutely thrombosed external hemorrhoids may be safely excised in the emergency department in patients who present within 48-72 hours of symptom onset.
Infiltration of a local anesthetic containing epinephrine is followed by elliptical incision and excision of the thrombosed hemorrhoid, its accompanying vein, and overlying skin.
Note: Simple incision and clot evacuation is inadequate therapy and should not be performed.
The incision should not extend beyond the anal verge or deeper than the cutaneous layer. A pressure dressing is applied for several hours, after which time the wound is left to heal by secondary intention.
In patients presenting after 72 hours from the start of symptoms, conservative medical therapy is preferable.
Different Types Of Hemorrhoids
Hemorrhoids are broadly categorized into two types, internal and external.
Internal hemorrhoids are located deep inside the anus, whereas external ones are fairly superficial and can be felt under the skin around the anal opening.
Internal hemorrhoids present in the anal canal can be symptomatic without prolapsing. In their normal state , hemorrhoids aid in stool control and are called anal cushions.
Anatomically, internal hemorrhoids have three anal cushions, which are located in the right anterior, right posterior, and left lateral areas within the anal canal.

When To Go To An Urgent Care Clinic
When you have a problem, do not wait too long to get medical care. If your problem is not life threatening or risking disability, but you are concerned and you cannot see your provider soon enough, go to an urgent care clinic.

Early Postoperative Complications Of Anorectal Surgery
Since many anorectal procedures can be performed safely and effectively in an ambulatory setting or an overnight stay, some patients may develop complications sooner or later after hospital discharge. Common early postoperative complications that bring patients back to the hospital include acute urinary retention, bleeding, fecal impaction and anorectal sepsis.
The incidence of acute urinary retention following benign anorectal surgery ranges from 0.5% to 17% depending on the extent of surgery, anesthetic technique, analgesic method, amount of intravenous fluid given and patients underlying disease. Bladder catheterization is the standard treatment of acute postoperative urinary retention. Should the volume of urine is less than 600 mL in low-risk individuals, the patients may be sent home without voiding. But if the catheterized urine volume exceeds 600 mL especially in high-risk patients , a self-retaining Foley catheterization may be required before discharge.

More recently, another option aimed at avoiding the postoperative pain of a hemorrhoidectomy has become available, called a stapled hemorrhoidopexy. A circular stapling device pulls the hemorrhoidal tissue upward and to its normal position, stapling it in place. The staples eventually fall out over time.
There are many options for the treatment of hemorrhoids. Its best to discuss the benefits and risks of each of these with your doctor in order to come up with the treatment plan that is best for your particular case.

The following are health problems in which it is recommended to go to the emergency room:
- Severe physical trauma, such as that caused by a car accident.
- Suicidal or homicidal feelings

Lifestyle Changes For Managing Hemorrhoids
You may help prevent hemorrhoids from forming or from worsening with some lifestyle changes. These changes include:
- Do not while moving your bowels.
- Limit the amount of time you spend on the toilet.
- Increase fiber in your diet. Fiber helps give stool bulk so it can be moved through your intestines, and it also helps water remain in the stool so it does not become dry and difficult to pass.
- Increase fluid intake. Drink six to eight glasses of water and other liquids to help keep your stool soft.
- Exercise regularly. This will help regulate and stimulate your bowels.
- Apply topical ointments. OTC creams and suppositories can help relieve pain and reduce swelling.
