Minor or minimally invasive hemorrhoid removal surgery is usually not painful. These procedures typically involve the use of a local anesthetic, which numbs the affected area. More severe or persistent hemorrhoids might require removal that is more invasive, such as hemorrhoidectomy, although these are done less often.
Symptoms Of Thrombosed External Hemorrhoids
External hemorrhoids can be felt as bulges at the anus, but they usually cause few of the symptoms that are typical of internal hemorrhoids.
External hemorrhoids can cause problems, however, when blood clots inside them. This is referred to as thrombosis. Thrombosis of such a hemorrhoid causes an anal lump that is
- very painful,
- tender, and
- often requires medical attention.
The thrombosed hemorrhoid may heal with scarring, and leave a tag of skin protruding in the anus. Occasionally, the tag is large, which can make anal hygiene difficult or irritate the anus. Even after such a hemorrhoid goes away, a residual skin tag may remain, and this may need to be surgically removed.
Whats The Procedure Like
Hemorrhoidectomy takes place in a hospital setting. Its usually an outpatient procedure, but in some cases, it may require an overnight stay.
Pre-op instructions will advise you when to stop eating, which is usually 6 to 8 hours before surgery. You wont be able to drive immediately, so be sure to arrange transportation home.
An IV will be inserted into your arm for medications to prevent infection, swelling, and bleeding. Youll need either general anesthesia or local anesthesia with sedation to help you sleep through the procedure.
Once youre asleep, the surgeon will insert a scope into the anal canal for closer examination of internal hemorrhoids. Theyll then cut the hemorrhoidal tissue with a scalpel or laser and close the wound with dissolvable sutures. This is called a closed hemorrhoidectomy.
In some cases, such as when theres a high risk of infection or the area is very large, the incision isnt sutured. This is called an open hemorrhoidectomy.
Another procedure, called hemorrhoidopexy, is somewhat less involved than hemorrhoidectomy. The surgeon uses staples to block blood flow, remove excess tissue, and lift hemorrhoids into a better position. This procedure comes with a greater risk of recurrence and rectal prolapse.
Once the surgeon is done, youll be moved to a recovery area where your vital signs will be monitored for several hours.

Surgical Treatment For Hemorrhoids
Today, many minimally invasive surgical options are available to treat hemorrhoids. Your physician will refer you to a surgeon if conservative treatments dont address your symptoms or if you arent a candidate for in-office procedures.
Common surgical procedures to treat hemorrhoids include:
- Hemorrhoidectomy. Large external hemorrhoids or prolapsed hemorrhoids may need to be removed by a surgeon.
- Hemorrhoid stapling. A special stapling tool can remove internal hemorrhoids and pull prolapsed hemorrhoids back into the anus.

Conventional Treatment For Hemorrhoids
When you go to see a doctor for enlarged hemorrhoids, he or she will look at your anus to see if its inflamed and whether the hemorrhoids come out of the anus when you push or if theyre already outside. Depending on the severity of your symptoms, there are a number of examinations your doctor may choose to perform in order to make a diagnosis. Doctors typically do a digital rectal examination first, which involves gently inserting a finger into the anus with gloves and lubricant. The doctor will feel the inside of the anal canal using circular movements, which allows him to examine the sphincter muscles and the texture of the membranes lining the anus.
If your doctor believes that you do have enlarged hemorrhoids, she will most likely perform a proctoscopy next. This involves inserting a short tube with a light and lens on it that exams the membranes lining the rectum. The doctor is looking to see if there are enlarged hemorrhoids and how big they are. People tend to fear the pain and discomfort that come with these procedures, but they usually dont hurt and do help your doctor make suggestions about how to get rid of hemorrhoids.

You can take a sitz bath in a few inches of water in a regular tub, but small tubs that fit over toilet seats are also available for this purpose at pharmacies. Following a sitz bath, gently pat the area dry to avoid further irritation.
- Use ice packs on the area to relieve swelling and pain.
- Don’t push too hard or strain while passing a bowel movement.

What Are The Treatment Options
Small hemorrhoids often clear up without treatment or with home treatment and lifestyle changes. If youve continued problems or complications, such as bleeding, you should see your doctor.
Your doctor can rule out other causes of bleeding during bowel movements, such as colon or anal cancer. If your symptoms become severe, your doctor may recommend medical procedures to remove or shrink the hemorrhoids.
Your doctor may recommend a minimally invasive outpatient treatment. Outpatient treatments are treatments that your doctor performs in their office. Some treatments include the following:
- Rubber band ligation is the most common nonsurgical treatment, and it involves your doctor tying a tight band around the base of the hemorrhoid to cut off the blood supply. Eighty percent of people who receive treatment for hemorrhoids have this type of treatment.
- During coagulation, your doctor uses infrared light, heat, or extreme cold to shrink the hemorrhoid.

Rubber band ligation can be used with first-, second-, and third-degree hemorrhoids, and may be more effective than sclerotherapy. Symptoms frequently recur several years later, but usually can be treated with further ligation.
The most common complication of ligation is pain. However, bleeding one or two weeks after ligation or bacterial infection in the tissues surrounding the hemorrhoids may occur.

Procedure For Prolapse And Hemorrhoids
PPH is also called a stapled hemorrhoidectomy. The doctor will use a stapler-like device to reposition the hemorrhoids and cut off their blood supply. Without blood, they’ll eventually shrivel and die.
