Hemorrhoids Treatment: Rubber Band Ligation

How to Treat External Hemorrhoids with Epsom Salt and Sitz Bath ?

The principle of rubber band ligation is to encircle the base of the hemorrhoidal clump with a tight rubber band. The tissue cut off by the rubber band dies, and is replaced by an ulcer that heals with scarring.

A similar procedure was described in 460 BC by the Greek physician Hippocrates:

And hemorrhoids you may treat by transfixing them with a needle and tying them with very thick and woolen thread, for application, and do not foment until they drop off, and always leave one behind and when the patient recovers, let him be put on a course of Hellebore.

Rubber band ligation can be used with first-, second-, and third-degree hemorrhoids, and may be more effective than sclerotherapy. Symptoms frequently recur several years later, but usually can be treated with further ligation.

The most common complication of ligation is pain. However, bleeding one or two weeks after ligation or bacterial infection in the tissues surrounding the hemorrhoids may occur.

Epsom Salt Sitz Bath For Hemorrhoids

Epsom salt can help with relieving the painful symptoms of hemorrhoids.

Epsom salt is a mineral compound that helps relax and heal the muscles and muscle related tension, which is why athletes often use this product.

Epsom salt can also help external hemorrhoids shrink and disappear more quickly.

While Epsom salt may not always result in a faster healing of hemorrhoids, it can help relax the rectal muscles, which can aid in relieving the discomfort when sitting.

You can add a cup of half of a cup of Epsom salt into a warm sitz bath and stir until it dissolves. You can also add other essential oils into the bath as well.

The Three Worst Things You Can Do

  • Spend too much time on the toilet. Having good bowel habits means eating a healthy diet and spending no more than five minutes on the toilet, says Dr. Nandra.
  • Over-wipe or wipe aggressively. Use plain, unscented, hypoallergenic wipes wet tissue cotton balls or a bidet to clean yourself.
  • Not call your doctor when it could be something more serious. Any kind of rectal bleeding should be checked out by a doctor, especially if it is associated with unintended weight loss or anemia, says Dr. Shah. These symptoms could be a sign of something more serious. Its important to listen to your body, and if something doesnt feel right, its best to get evaluated by a medical professional.

    Thrombosed Hemorrhoids Sitz Bath

Lifestyle and dietary changes are the best way to prevent hemorrhoids. Staying physically active and eating a nutritious, balanced diet help keep your bowel movements regular.

Eat lots of high fiber foods and drink plenty of water to keep the digestive process moving correctly and prevent constipation. Regular exercise and avoiding sitting for long periods of time can also help prevent hemorrhoids.

The most effective way to avoid constipation is to go to the bathroom when you first feel the urge. Delaying a bowel movement allows the bowel to reabsorb water from the stool. This makes stool harder when you finally do go.

  • Fill The Sitz Bath With Warm Water

Let the water from the faucet run until it is quite warm. You want it to feel warm enough to get the blood flowing but not so hot that it makes your skin red. A good test is to let the water run over your wrist. The skin there is thin and very sensitive.

One the temperature is right, fill the tub or basin and swirl in any added ingredients. If you’re using a portable basin, do not overfill it as you still need to carry it to the toilet bowl.

Once in place, top the basin up with a couple of glasses of warm water. Some people use a thermos bottle if the bathroom sink is far away. This saves you from having to get up if the temperature is not right or needs warming up.

    Symptoms Of Thrombosed External Hemorrhoids

External hemorrhoids can be felt as bulges at the anus, but they usually cause few of the symptoms that are typical of internal hemorrhoids.

External hemorrhoids can cause problems, however, when blood clots inside them. This is referred to as thrombosis. Thrombosis of such a hemorrhoid causes an anal lump that is

    • very painful,
    • tender, and
    • often requires medical attention.

The thrombosed hemorrhoid may heal with scarring, and leave a tag of skin protruding in the anus. Occasionally, the tag is large, which can make anal hygiene difficult or irritate the anus. Even after such a hemorrhoid goes away, a residual skin tag may remain, and this may need to be surgically removed.

    Symptoms Of Prolapsed Internal Hemorrhoids

Hemorrhoids prolapse when their blood vessels swell and extend from their location in the rectum through the anus. In the anal canal, the hemorrhoid is exposed to the trauma of passing stool, particularly hard stools associated with constipation. The trauma can cause bleeding and sometimes pain when stool passes.

    The presence of

    • inflammation, and
    • constant moisture

can lead to anal itchiness , and occasionally the constant feeling of needing to have a bowel movement. The prolapsing hemorrhoid usually returns into the anal canal or rectum on its own or can be pushed back inside with a finger, but falls out again with the next bowel movement.

What Are The Signs And Symptoms Of An Internal Or Prolapsed Hemorrhoid

Most often, internal hemorrhoids have no symptoms but are only found if there is bleeding with a bowel movement or if the hemorrhoid prolapses so that it can be felt outside of the anus. This may lead to itching and pain as well as the bleeding.

Prolapse of an internal hemorrhoid occurs when the internal hemorrhoids swell and extend from their location in the rectum through the anus. A prolapsed internal hemorrhoid:

    • Can be felt as a lump outside the anus
    • Can be gently pushed back through the anus, this may resolve the location of the hemorrhoid, but does not fix the hemorrhoid itself
    • May enlarge and swell even more if it cannot be pushed back
    • May become entrapped, which requires more urgent medical attention

Hemorrhoids may also cause anal itching , and a constant feeling of needing to have a bowel movement .

Internal hemorrhoid severity can be graded:

    • Grade I: Prominent blood vessels with no prolapse
    • Grade II: Prolapse with bearing down but with spontaneous reduction
    • Grade III: Prolapse with bearing down but requiring manual reduction
    • Grade IV: Prolapse with inability to manual reduction

The hemorrhoid treatment consists of soaking the buttocks and hips in a solution of saline or plain water. Depending upon the condition being treated, hot or cold water may be used. One of the main effects of a sitz bath is to increase blood flow to the abdomen and pelvis, which can help in the treatment of hemorrhoids as well as other medical conditions.

    Uses of Sitz Baths

One of the most popular uses of the sitz bath is as a means to alleviate the symptoms of hemorrhoids. Many hemroids sufferers find sitz baths a very effective means to reduce swelling and inflammation in hemorrhoidal tissue. Consequently, sitz baths can help relieve the pain which often accompanies hemroids.

Sitz baths have many other uses besides the treatment of hemorrhoids. Sitz baths can be used to relieve the pain associated with various types of inflammatory conditions in the pelvic area, genital herpes and prostate problems being a couple of examples. The treatment can also relieve the pain of certain types of muscle disorders, such as ovarian and uterine cramps in women. Sitz baths have long been thought to renew vigor and good health.

What Are Two Types Of Hemorrhoids

Hemorrhoids are enlarged, swollen veins in the lowest part of the rectum and anus. These veins bulge and get irritated, especially when the patient defecates, and cause bleeding. Hemorrhoids are also commonly called piles. It is one of the most common causes of rectal bleeding. Hemorrhoids occur in adults. It can cause significant pain and discomfort if left untreated.

There are two types of hemorrhoids:

    • Internal hemorrhoids: They are present deep inside the rectum. They are usually not seen externally or felt and are painless.

REVIEW OF sitz baths for hemorrhoids

Before you begin, it is important to clean the tub/basin. This is true regardless of whether you are using a bathtub or a basin product. If using a basin, make sure it is firmly secured once cleaned.

A sitz bath should be performed twice a day, ideally right after you have a bowel movement. One of the reasons for this timing is that a sitz bath can sometimes relax the anal canal enough to induce a bowel movement. Due to how connected constipation and hemorrhoids can be, this result may actually be desirable. In any case, should this happen you will need to thoroughly clean the tub or basin and empty out the water as soon as possible after passing the stool.

Patient information: A handout on hemorrhoids is available.

Hemorrhoids develop when the venous drainage of the anus is altered, causing the venous plexus and connecting tissue to dilate, creating an outgrowth of anal mucosa from the rectal wall. However, the exact pathophysiology is unknown. Hemorrhoids occur above or below the dentate line where the proximal columnar transitions to the distal squamous epithelium . The anus is approximately 4 cm long in adults, with the dentate line located roughly at the midpoint.2 Hemorrhoids developing above the dentate line are internal. They are painless because they are viscerally innervated. External hemorrhoids develop below the dentate line and can become painful when swollen. The extent of prolapse of internal hemorrhoids can be graded on a scale from I to IV, which guides effective treatment . This grading system is incomplete, however, because it focuses exclusively on the extent of prolapse and does not consider other clinical factors, such as size and number of hemorrhoids, amount of pain and bleeding, and patient comorbidities and preferences.3

What Is The Role Of Warm Baths In The Treatment Of Hemorrhoids

Bathing in tubs with warm water universally eases painful perianal conditions. Relaxation of the sphincter mechanism and spasm is probably the etiology. Ice can relieve the pain of acute thrombosis.

Some authors do not suggest mechanisms such as the sitz bath for symptom relief. The rigid structure of these portable bathing apparatuses can act in a similar fashion as a toilet seat, causing venous congestion in the perianal area and potentially exacerbating the problem. However, sitz baths do have a role in the treatment of older or immobile patients who cannot routinely get in and out of a bathtub.
