For many patients, hemorrhoids are merely an annoyance and will resolve on their own with simple care. But when a hemorrhoid becomes serious enough, a patient will come to your office seeking treatment and perhaps surgery.
But when will a hemorrhoid require surgery? There are several factors you should take into consideration based on the type of hemorrhoid the patient has and the types of treatment options used in the past.
Medical Treatment For Your Hemorrhoids
Dr. Rivas develops personalized hemorrhoid treatment plans after completing a thorough examination. The treatments we recommend may depend on the type of hemorrhoids, as well as your symptoms. Initially, we may suggest lifestyle changes to ease your pain, such as increasing your intake of fiber and water to improve bowel movements and sitz baths to alleviate pain and discomfort.
However, when at-home treatments fail, Dr. Rivas may suggest a procedure to destroy or remove your hemorrhoids, such as:
- Rubber band ligation
- Infrared, laser, or bipolar coagulation
- Thrombectomy

Surgery For External Hemorrhoids
When surgery is warranted, most external hemorrhoids are surgically cut out or excised. This is called a hemorrhoid excision.
Alternatively, an incisional hemorrhoidectomy may be performed for a thrombosed external hemorrhoid. With this type of surgery, a cut is made over the skin of the hemorrhoid. Just the clot is removed.
An incisional hemorrhoidectomy is less likely to be successful if a patient has been experiencing pain for more than 48 hours. In addition, if the incision is too small, blood can reaccumulate and form another clot inside the hemorrhoid.
Do Prolapsed Hemorrhoids Hurt
Prolapsed hemorrhoids tend to be more painful when sitting as opposed to standing or lying down. They may also hurt more during a bowel movement.
Prolapsed hemorrhoids can be especially painful if a blood clot has formed within the hemorrhoid. This is known as a thrombosed hemorrhoid.
A thrombosed hemorrhoid isnt as dangerous as a blood clot in your heart, for example, but it can be very painful. A thrombosed hemorrhoid may need to be lanced and drained to relieve the pain.
A prolapsed hemorrhoid may also be extremely painful if its strangulated, which means the blood supply to the hemorrhoid has been cut off.

What Should I Expect? Hemorrhoids After Hysterectomy Surgery
Before the Surgery: Your doctor will give you a regimen of things to do to prepare for the surgery. This will probably include directions about what and how late you can eat the day before. Your bowels will need to be emptied prior to the surgery, so you will be instructed on how to do this. Because you will be placed under anesthesia during the surgery, you will also need to have someone drive you home later. You will be asked to arrive at the outpatient center by a certain time to be prepped for surgery.
During the Surgery: This depends on the type of surgery you receive. In any type, you will be placed under anesthesia to eliminate pain during the procedure. You may or may not be awake during the procedure.
If you have the stapled hemorrhoidectomy, the doctor will lift the hemorrhoid tissue back in place and then use a circular stapling tool to staple it.
If you have a conventional hemorrhoidectomy, the doctor will
- Make an incision in the hemorrhoid tissue.
- Remove any blood clots.
- Remove the hemorrhoid blood vessels.
- Remove the hemorrhoid.
Some doctors use sutures to close the area. Others think healing is more successful when the wound is left open. The doctor will pack the wound with gauze to absorb bleeding.
You may experience bleeding with your first few bowel movements. This is not a cause for worry.
When To Seek Medical Advice
See your GP if you have persistent or severe symptoms of haemorrhoids. You should always get any rectal bleeding checked out, so your doctor can rule out more potentially serious causes.
The symptoms of haemorrhoids often clear up on their own or with simple treatments that can be bought from a pharmacy without a prescription . However, speak to your GP if your symptoms don’t get better or if you experience pain or bleeding.
Your GP can often diagnose haemorrhoids using a simple internal examination of your back passage, although they may need to refer you to a colorectal specialist for diagnosis and treatment.
Some people with haemorrhoids are reluctant to see their GP. However, theres no need to be embarrassed, because GPs are very used to diagnosing and treating haemorrhoids.
