Sometimes the symptoms of enlarged hemorrhoids are so bad that treating the symptoms alone is no longer enough. Then there are various procedures that can be done to remove the tissue that is causing problems.

Pain After Hemorrhoid Treatment

It is common to have pain in the week following treatment. Some procedures are more painful than others. Your pain may be significant or you may simply feel discomfort.

A hemorrhoidectomy usually results in more pain than sclerotherapy. Hemorrhoid banding usually results in much less discomfort.

Your level of pain will also depend on how severe your hemorrhoids were, and whether your stools stay soft while you are recovering. You may also have pain with urination.

Sclerotherapy of a small, single hemorrhoid may result in minor pain. A hemorrhoidectomy of multiple very large hemorrhoids may result in significant pain.

No matter which treatment you have, it is normal to have pain with a bowel movement in the week after surgery. Straining and pushing can make the pain much worse.

When Is Hemorrhoid Surgery Necessary

Hemorrhoid surgery may be necessary for internal hemorrhoids that have prolapsed if this prolapse is significant, such as when the protruding tissue cannot be pushed back in. Patients With Compounding Conditions

Pregnant patients often develop hemorrhoids as intra-abdominal pressure increases, particularly during the third trimester.36 Also, acute episodes of pain and bleeding are common in pregnant women with preexisting hemorrhoids.

Conservative treatment is the main approach in pregnant patients because most hemorrhoids regress after childbirth. This includes increased dietary fiber, stool softeners and sitz baths, which are safe to use for external hemorrhoids. Any office-based or surgical intervention should be postponed until after childbirth, if possible. Kegel exercises and lying on the left side are also recommended to relieve symptoms. In cases of severe bleeding, anal packing appears to be useful.

Immunosuppressed patients and those on anticoagulant therapy are more prone to serious complications such as sepsis and profuse bleeding. Thus, conservative management should be used in these patients as well. Injection sclerotherapy may be beneficial, as it has been shown to decrease bleeding. Of note, patients on immunosuppressive agents should stop taking them and start taking an antibiotic, and patients on anticoagulant or antiplatelet medications should be instructed to stop them one week before any intervention.

The procedure typically causes the hemorrhoid to shrink and fall off within about a week, though several short visits to your doctor may be necessary to completely get rid of the hemorrhoid. Minor bleeding and a feeling of pressure may occur, but this can usually be alleviated with OTC pain relievers. This procedure is not associated with significant recovery time.

External Hemorrhoid Itch Relief

External hemorrhoids can become extremely itchy. The itching may even affect your quality of life.

There are ways to relieve and soothe the symptoms of itching. One way is to have a sitz bath.

A sitz bath involves sitting in a few inches of warm water for 15 minutes several times a day.

This reduces swelling that causes itching.

Witch hazel for hemorrhoids and over-the-counter hemorrhoid creams can be used to reduce the symptoms of itching, as well.

Additionally, applying a cold pack or ice pack like hemorrwedge can numb the area and bring down swelling that causes itching.
