Hemorrhoids are painful, unpleasant, and, um, well, difficult to talk about. But they actually are quite common: about half of people over age 50 have had them. However, theyre easy to treat and manage.

Hemorrhoids can be troublesome and embarrassing, but they often shrink on their own with simple self-help care and over-the-counter remedies, says Dr. Howard LeWine, assistant professor of medicine at Harvard Medical School.

How To Treat An External Thrombosed Hemorrhoid

External hemorrhoids come with a great deal of pain and discomfort. To make matters worse, some external hemorrhoids can become thrombosed. A thrombosed external hemorrhoid is an external hemorrhoid that has developed a blood clot.

Thrombosed external hemorrhoids are characterized by their symptoms and their look. They appear as a hard lump that is blue or purple in color. The color is a result of the blood clot.

Thrombosed external hemorrhoids can form as a single pile or a circle of piles. They are classified as grade IV hemorrhoids .

The cause of thrombosed hemorrhoids is not well known.

Some doctors feel that events trigger the development of thrombosed external hemorrhoids:

  • Physical exertion

The main course of treatment for a thrombosed hemorrhoid is an external thrombectomy.

The surgical procedure involves placing a small cut in the clot and draining it.

When Hemorrhoid Surgery May Be Necessary

Very few people fewer than 10 percent of all adults who see a doctor because of symptomatic hemorrhoids will require a surgical operation.

Surgery to remove hemorrhoids is called a hemorrhoidectomy.

Your doctor may recommend a hemorrhoidectomy if:

  • You have large external hemorrhoids

It can also be done with a spinal anesthetic block while you are awake , or with a combination of relaxing and sedating drugs given intravenously and a local anesthetic injected around the anus.

Hemorrhoidectomy is associated with postoperative pain, but the procedure is successful for 95 percent of cases.

More recently, another option aimed at avoiding the postoperative pain of a hemorrhoidectomy has become available, called a stapled hemorrhoidopexy. A circular stapling device pulls the hemorrhoidal tissue upward and to its normal position, stapling it in place. The staples eventually fall out over time.

There are many options for the treatment of hemorrhoids. Its best to discuss the benefits and risks of each of these with your doctor in order to come up with the treatment plan that is best for your particular case.

How Are External Hemorrhoids Diagnosed

To diagnose external hemorrhoids, a doctor will ask about your health history and symptoms. They will also likely do a physical exam.

In the case of external hemorrhoids, they will generally be able to see the hemorrhoids by examining the area around your anus.

They may also perform a digital rectum exam and anoscopy to check for issues inside your anal canal and rectum, including internal hemorrhoids.

During a digital rectal exam, your doctor will insert their gloved, lubricated finger into your anus. Theyll be able to feel abnormalities inside.

During an anoscopy, the doctor will insert a device called an anoscope into your rectum that allows them to see abnormalities.

If you also have rectal bleeding, your doctor will likely request that you get a sigmoidoscopy or colonoscopy to rule out colorectal cancer. These methods will also detect any internal hemorrhoids.

See a doctor if your hemorrhoids dont get better after a week of treatment or if you experience bloody stools or rectal bleeding. Your doctor will determine if the bleeding is from hemorrhoids or a more serious condition.

Rectal bleeding can happen due to colorectal or anal cancer, especially if its accompanied by changes in your bowel habits, including the color and consistency of your stools.

If you have a hemorrhoid that develops rapidly or is extremely painful, it could be a sign of a blood clot that has developed inside. For the most relief, you must remove the clot within the first 48 hours.

Exercise regularly. Even brisk walking 20-30 minutes every day can help keep you from getting stopped up.

Use a pillow. Sit on a cushion instead of a hard surface. It will ease swelling for any hemorrhoids you have. It may also help prevent new ones from forming.

Take breaks. If you must sit for a long time, get up every hour and move around for at least 5 minutes.
